The pocket sized 4-panel 2-sided schedule is 5 5/8" x 3 3/5" (143mm x 95mm) folded and folds out to a size of 7 1/4" x 11" (184mm to 279mm) . It was printed in the USA.

Condition: the schedule has a small 1/2" x 1/4" tar/hole on the front panel's bottom section near the "Wells Heating Oils" ad (see photos).
