Matchbox Heroes Series, no. 91375, diecast metal USA Ambulance with opening rear doors (and patient on stretcher inside), red fenders grille and chassis, dark blue windows, American flag on front doors, red white and blue "USA Ambulance" graphic on sides, Match box logo on side near rear, and four dot chrome wheel hubs.

The Heroes Series was a special patriotic themed series issued by Matchbox in 2001 as a tribute to the 9/11 first responders. The back of the card has a checklist of safety tips for children to follow in an emergency and instructions on how to reach a first responder by calling 9-1-1.

Made in China in 2000. The rescue vehicle is 3" / 76mm long.

The ambulance is in mint condition.  The cardboard blister card is in very good plus condition with light shelf wear on the bottom right corner edges (see photos).
