The card is unused but it has a small 1/2" vertical tear near the center of the top edge (see photos).
The 21 story 495 room luxury hotel was built in 1926 and was a favorite of celebrities and musicians like Lillian Gish, Frank Sinatra, Muhammad Ali, Jimi Hendrix, Olivia Newton-John, Led Zeppelin, The Who, and many others. The building was demolished in 2006 to make room for a 1,398 foot tall skyscraper.
